I need a design for a health management website that connects doctors to  patients.  The website will consist of: 
A main page  mainly describing our goal, services, and initiatives towards (Who we are AKA "About Us", Our services,  Contact Us, Quick Links for health tips, Language selection,  Find a doctor, search bar,  Member Login Section, small glossary section below with links to all entities mentioned prior, Help section (Helpful tips and tutorials, Used to request help from admin, customer service, to resolve issues or get assistance). 

A profile page with two types:

1) Doctor Profile: Doctor Personal info (First, Last, Middle Name, Title), Clinic, Hospital Info, Department, Credentials,  Years of Experience, Patients rating, #Patients seen during the current week, #Patients seen in total,  Picture, Link to email doctor, Link to print doctor's profile). Tabs for "About Me", "Contact Info", "Ratings & Comments", "Documents" (To be downloaded and uploaded by patient), "Patients (List of Patients alphabetical order by name)", "Calendar" (to see doctor's availability for booking) underneath all links. 

2) Patient profile: 
About me, Personal Info, Height, Weight, Blood Type, Age, DOB, Name, etc..., Message and email link.
Picture of patient with menu made of "Personal Data", "Biometrics", "Allergies", "Illnesses", "Medications", "Medical Procedures", "Doctors" treating the patient, "Documents" (To be downloaded and uploaded by patient). Underneath all, other patients he or she is related to (For example: family member, so patient can't connect to other patients who are family members)

3) Login section: 
New Users will have option to register using:
First Name (Required), Last Name, Middle (not required), Address (Not required),  City (Required), Country (Required), Email (Not required), Phone Number (Required), Choose UserName, Password, Account Type (Doctor or Patient). Once registered, they will log in using UserName, Password, and Account_Type.
Curent Users will log in using UserName, Password, and Account_Type and option to retrieve username or password.
